This includes conducting patient assessments and providing safe and effective telephone advice, care instructions, and direction to the appropriate level of care. The Triage Registered Nurse works collaboratively with members of the interdisciplinary team to provide exceptional customer service and achieve high-quality patient care standards. Schedule: 3:00 p.m.–11:00 p.m., four days per week, primarily working as the Triage RN, with the flexibility to work some shifts in the community as needed. Weekend coverage is required every fourth weekend from 8:00 a.m.–8:00 p.m. in Triage, which may be worked from home. The position also includes an on-call rotation of approximately 10–15 shifts per year and coverage for approximately 2–3 holidays per year. Key Job Duties Telephone triage and follow-up to patient care-related contactsResponsible for providing skilled nursing assessment and interventionProvide updates to the interdisciplinary team and attending physiciansUpdate Plan of Care as necessaryProvide education to patients and family members on symptom management, disease progression, medication, and other associated patient care needsRecord and maintain complete and accurate records of patient and physician communicationsFax/call new and refill prescriptions according to physician ordersRecord all received orders to the Electronic Medical Record and initiate follow-up as neededRespond to patient needs according to medical priority.Provide assistance and direction to patient care staff as neededCoordinate volunteer deliveriesAbide by Standards of Nursing Practice as outlined in Wisconsin Administrative CodeComply with all applicable local, state and federal laws and regulations as well as policies and procedures of UniSatisfy mandatory training requirements regarding local, state and federal regulations and/or Unity's policies and proceduresConsistently and concisely document activity according to department protocols and standardPromote and maintain confidentialityPerform other job-related duties as assignedMaintain regular and reliable attendanceShares in providing seven days-a-week coverage Qualifications Current unrestricted Registered Nurse licensure from the State of WisconsinMinimum of two years' experience as a professional nurse; one to three years of hospice experience preferredExcellent critical thinking, interpersonal relationship building, and technical skillsValid Wisconsin Driver's License, reliable transportation, and proof of current automobile insurance coverageKnowledge, Skills, and Abilities:Computer proficiency (MS Office – Word, Excel, and Outlook)Excellent critical thinking, interpersonal relationship building, and technical skillsHighly motivated and organized with a strong attention to detail and analytical skills to help solve problems.Ability to communicate effectively, both verbally and in writing.Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment.Ability to keep up to date with policies, practices, and regulations.Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures.Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity's policies and procedures.Perform other job-related duties as assigned.Ability to adjust to frequently changing demands.
